PROCEDURE FOR APPLYING FOR A MORTUARY PASS

Since 28 November 2025, the procedure for applying for a Mortuary Pass has been exclusively online via the official consular services portal, accessible via the following link:

https://evisacam.cm/ords/dl_portal/r/public_portal/home

The procedure must be followed strictly in accordance with the six (06) main steps below:

1-    Open a user account for new users (create a username and password) or log in to the platform using a username and password for users who already have an account;

2–    Select the requested service by clicking on the ‘services’ tab to display the options and choose the one relating to the Mortuary Pass;

3–    Fill in the online service request form, providing all the mandatory and useful personal information required by the system;

4–    Upload all the required documents listed below;

5–    Pay the fees for issuing the Mortuary Pass directly online on the platform. Once payment has been made, proof of successful transaction will be generated in the form of a code or payment receipt to be printed and kept for presentation to the Embassy when collecting the requested document;

6–    Document collection. This is done upon presentation of the online pre-registration form bearing a QR code. This document must be sent to the Diplomatic Mission by post, accompanied by a pre-addressed return envelope in which the issued document will be returned to the applicant.

LIST OF DOCUMENTS TO BE PROVIDED FOR A REQUEST FOR A MORTUARY PASSE

  1. Online pre-registration stage

During this stage, the following MANDATORY documents must be properly SCANNED and uploaded to the application:

– Authorization for the transfer of the body;

– Burial permit;

– A copy of the death certificate;

– A copy of the identity page of the deceased’s passport or a certificate of loss and/or damage or a copy of their birth certificate;

– A document containing information relating to embalming (letter from the funeral home).

– A document containing information relating to the seals (letter from the funeral home).

ISSUANCE AND COLLECTION OF THE MORTUARY PASSPORT

Once the online procedure has been completed and the application for a Mortuary Passport has been approved, the travel document is produced and sent to the funeral home using the return envelope provided for this purpose.

The return envelope in question is a prepaid envelope with a tracking number and clearly marked with the recipient’s full address (Canada Express Post; UPS, FedEx; Purolator, etc.).

The Mortuary Pass is valid for ninety (90) days from the date of issue.

IN THE EVENT OF LOSS OF THE DEATH PASS

Given the significant physical changes to this document, its replacement follows the same process as described above. However, instead of the passport, a CERTIFICATE of loss must be submitted, issued by the police services of the place where the event occurred. As with the passport, this is the physical report detailing the circumstances of the loss of the document and NOT the document bearing only the file number.

If a previously issued Mortuary Pass has not been used, proof of the reasons for this must be included in the application for a new pass.

IN CASE OF PROBLEMS RELATED TO THE DEATH CERTIFICATE APPLICATION PROCEDURE

Two scenarios are possible:

  • The online application for a Death Certificate is incomplete. It will be returned once or twice for modifications and/or to complete the missing documents. The applicant will receive an email notification detailing the steps to be taken to correct the problem. The applicant must:
  • Return to the consular services platform;
  • Click on ‘modify my application’ and follow the steps;
  • Correct the problem according to the instructions contained in the return notification;
  • Resubmit their application.
  • Rejection of the application for a Mortuary Pass. This occurs in cases where the user does not meet the requirements, or the file has been returned several times and has not been completed and/or corrected as required. In this case, the applicant will have to start the entire procedure again and pay the relevant fees online once more.

For any other technical issues related to the online procedure (document uploading, payment difficulties, etc.) and requests, please send an email to the following address: contact@evisacam.cm.

The message must be BRIEF and contain the following information:

  • The subject of the request;
  • The identification number on the pre-enrolment form;
  • Proof of payment (receipt);
  • Once the issue has been submitted, wait for a response from the Evisacam team.

In the event of double payment, requests for refunds of the excess amount should be sent to htts://claim.evisacam.cm/.

N.B: Only duplicate payments attributable to a possible malfunction of the consular services platform will be refunded after verification of the documents submitted in support of the request.

Multiple payments made through the successive submission of several requests of the same nature by the same person will not be refunded.
